Form IPR Guidelines
The Individual Participant Request (Form IPR) provides an avenue for
applicants to earn CE points through non-MLA- approved CE activities. Be
aware that activities such as MLA journal clubs require submission of
a completed IPR form.
Form IPR is available on the academy forms page or directly from MLA headquarters.
Completing Form IPR
Completing the IPR form is straightforward, but requires completion
of all parts of the form. The portfolio reviewer will use the information
provided on this form to determine if the activity will be awarded points.
Identifying Information
- your name
- title of the activity
- date(s) of the activity
- location of the activity (institution, city, state)
- contact hours, not including lunch or breaks
Description of Course Content
Provide the following information
about the activity (copies of program announcements/course materials can
be used to address any or all of these issues)
- course/activity objectives
- program content and time schedule
- intended audience
- means of assessing achievement (pre-test, post-test, etc.)
- means of assessing satisfaction
- instructor's name and affiliation
Relevance to Health Information Science
Describe why this
activity was relevant to your professional assignment and/or your goals
of professional growth and development.
Documentation of Accomplishment
Include evidence you have completed the entire activity described above.
This evidence can be a copy of a certificate of completion or a letter
from the instructor or the organization that sponsored the activity.
